In healthy women, menstruation sets in approximately between the ages of 12 to 14 years, and persists throughout the reproductive period of life with an average rhythm of 28 days and the duration of flow between 4 and 6 days.
Disorders of menstruation are amongst the commonest of gynecological complaints. These are often symptomatic, and inspite of meticulous search, the cause remains undetermined.
The classification of menstrual disorders is below.
1. Precocious menstruation.
If menstruation starts before the child reaches the age of 10, the condition is referred to as precocious menstruation.
It is defined as absence of menstruation.
It is again divided into two, basing on different causes.
A. Primary amenorrhea is defined when menstruation fails to begin by the age of 16, or in presence of well developed secondary sexual characters, menses do not start by the age of14.
B. Secondary amenorrhea is defined as amenorrhea of 6 months or more in a women with previous normal menstruation, in absence of pregnancy.
3. Infrequent and scanty menstruation.
In this condition, the intermenstrual cycle is prolonged beyond the normal 28 days. In some patients menstruation lasts one or two days and the loss is also mere.
4. Premenstrual tension.
Many women experience premenstrual symptoms 7-10 days before the onset of bleeding; irritability, lassitude, malaise, headache, fullness of breasts, abdomen, face and feet, constipation etc. All these together termed as Premenstrual tension.
Dysmenorrhea or painful menstruation, is one of the most frequent gynecological problem.
A normal menstrual blood loss is 50 - 80ml, and does not exceed 100ml. ln menorrhagia the menstrual cycle is unaltered but the duration and quantity of the menstrual loss are increased.
In polymenorrhoea or epimenorrhoea the menstrual cycle is reduced from the normal of 28 days to a cycle of 2-3 weeks and remains constant at that frequency. The combination of menorrhagia and epimenorrhoea is termed as epimenorrhagia.
Metrorrhagia should be defined as irregular, acyclical bleeding from the uterus. The bleeding may be intermittent or continuous.
